Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• The Ashtabula County VA Clinic offers general medical care - including primary care - behavioral health - diabetes care - laboratory services - nutrition services - and more
• The City of Ashtabula is located on Lake Erie's coastline about an hour between Cleveland - Ohio and Erie - Pennsylvania
• Major duties include but are not limited to: Provides direct patient care full time and works as a member of an expanded healthcare team (social worker - dietician - clinical pharmacists - behavioral health clinician - and a care manager) to oversee and coordinate medical care for a population of Veterans in a group practice setting utilizing the PACT model
• Provides comprehensive care for Veterans for a panel size of approximately 900-1000 patients or serves walk-ins
• Performs initial histories and physical examination on outpatients using CVT technology and/or face-to-face visits
• Provides ongoing outpatient care for assigned patients
• Screens outpatients to determine the need for further care
• Prescribes medications within defined scope of practice
• Orders diagnostic studies such as laboratory tests - x-rays - electrocardiogram
• Reviews need for and orders health promotion and disease prevention measures
• Initiates - responds to and expediates requests for consultations - special tests and studies Provides education and counseling of patients and families in preventive care - medical conditions - and use of prescribed treatments and drugs
• Attends staff meetings - team meetings - some committee meetings - and completes some peer review evaluations
